Required Health Forms
We have recently been informed that each marcher needs a health form completed to be able to travel with the Marching Band for competitions and overnight trips.
We have recently been informed that each marcher needs a health form completed to be able to travel with the Marching Band for competitions and overnight trips.
- Please fill out the following health form and return to Mr. Pearman by August 29th.
- These forms are taken to all competitions and overnight trips.
- Medications will not be carried by staff/adults for competitions.
- Medications will be carried by staff/adults for the overnight trip.
- For the overnight trip, if a student is needing a daily medication, a medication form also needs to be filled out and signed by a doctor, this is a form that will be turned into the school nurse, if you have already done this, Mr. Pearman will get this information from the school nurse.
- Students can carry emergency medications themselves, but forms still need to be filled out and turned in.
